Our Community Nursing services are designed to support NDIS participants with ongoing health needs, chronic conditions, and complex care plans in a way that promotes independence, dignity, and wellbeing.

What We Offer
Community Nursing through Amaze ConnectCare will include:
- Wound care & pressure injury management
- Continence assessment & catheter care
- Medication administration & monitoring
- Diabetes management (including insulin support)
- Stoma care & PEG feeding
- Post-hospital care & recovery support
- Health assessments & risk planning
- Support with chronic and complex conditions
All care will be tailored to your health goals and delivered in coordination with your support network and GP or treating team.

FAQs
Q: Do I need a referral from my doctor?
A: In some cases, yes. For complex care (e.g. PEG, wound care), a clinical referral or history may be required to ensure safe handover.
Q: Will I have the same nurse regularly?
A: We aim to provide continuity of care wherever possible so you can build trust and familiarity with your nurse.
Q: Is Community Nursing covered in my NDIS plan?
A: Yes, if you have funding under the Capability Building Daily Activities – Nursing Care category. If you’re unsure, we can help you check.

Our Team
Our Community Nursing team consist of:
- Registered Nurses (RNs) with AHPRA registration
- Enrolled Nurses (ENs) under supervision
- All with current NDIS Worker Screening Checks, CPR/First Aid certification, and relevant clinical experience
