 Role Purpose

As a General Support Worker at Amaze ConnectCare, you will assist NDIS participants in their homes and communities to enhance their independence, access, and quality of life. This includes personal care, transportation, household support, and social engagement, all tailored to their individual goals and NDIS plan.

Key Responsibilities

  • Deliver daily living supports, including personal care, hygiene, and dressing
  • Support participants with domestic tasks such as meal prep, cleaning, and laundry
  • Provide safe, respectful transport to appointments, shops, and community events
  • Promote and support community participation and social engagement
  • Use assistive products appropriately and ensure safe handling
  • Follow participant care plans and support documents
  • Maintain professional boundaries and confidentiality at all times
  • Complete incident reports and progress notes accurately and promptly

Essential Requirements

  • Demonstrated experience in disability, aged care, or community services
  • Current NDIS Worker Screening Check
  • Current Working With Children Check (WWCC) – valid in Victoria

Role Purpose

As a Registered Nurse with Amaze ConnectCare, you will provide essential clinical care to NDIS participants in their homes or community settings. This includes wound care, catheter management, medication administration, and health assessments. You will support participants with complex or ongoing medical needs in line with NDIS-funded care plans.

Key Responsibilities

  • Deliver safe and evidence-based clinical nursing care
  • Complete wound management, catheter care, PEG feeding, and complex medication administration
  • Conduct nursing assessments and contribute to care planning
  • Monitor participant well-being and escalate clinical concerns when required
  • Document clinical interactions, outcomes, and care notes accurately

  • Develop and implement individualised exercise plans to improve participant mobility and functional independence

Role Purpose

As a Home Modification Technician or Builder with Amaze ConnectCare, you will support NDIS participants by making practical adjustments to their home environments to enhance safety, accessibility, and independence. You will work closely with occupational therapists and our team to scope, quote, and complete modification works.

Key Responsibilities
  • Assess and complete minor to moderate home modifications, including ramps, rails, and bathroom adjustments
  • Collaborate with occupational therapists and participants to ensure modifications meet NDIS goals
  • Provide accurate quotes, timelines, and material requirements
  • Comply with building regulations, standards, and work health and safety practices
  • Maintain clean and safe work environments in participant homes
  • Document work completed and submit before-and-after records where required
  • Use respectful communication with participants, families, and team members

  • Trade qualification or proven experience in carpentry, renovations, or building works

This includes AAC devices, switches, tablets with accessibility software, and other related devices. You will work closely with allied health professionals and participant support teams.

Key Responsibilities
  • Assist in the assessment, selection, and implementation of communication and information access devices
  • Work collaboratively with Speech Pathologists, OTs, and families to identify appropriate technology solutions
  • Support trial setups, training, and documentation for assistive technology recommendations
  • Liaise with suppliers to ensure suitable equipment configuration and timely delivery
  • Maintain knowledge of the latest accessibility tools, devices, and NDIS funding categories
  • Document recommendations and outcomes, provide ongoing tech support to participants and teams
Essential Requirements
